Step one:

Start by creating a new document in Photoshop. Leave all the default settings except for the canvas dimensions, use 700×400 pixels with a black background. Now make a new layer set and name it ‘Text Design’.

Select your Horizontal Type Tool, above your screen under the options palette set the font family to Minion, regular, 72pt, strong and #A3A3A3 color shade. In a new text layer type your text.

Step two:

Under Layer Style(Layer > Layer Style) add an Inner Shadow, Gradient Overlay, Pattern Overlay and Stroke blending options to your text layer.

Step three:

In a new text layer type out your name in white color shade.

Step four:

Under Layer Style(Layer > Layer Style) add an Inner Shadow, Inner Glow and Stroke blending options to your white text layer. Then set the layer’s blending mode to Darken.

Step five:

Duplicate the ‘Text Design’ layer set and go to Layer > Merge Layer Set. You should now have a new layer with the text design. Turn off the original ‘Text Design’ layer set, you will need it later on.

Step six:

Now make a copy of your Text Design layer and apply a motion blur through Filter > Blur > Motion Blur, use the settings below.

Step seven:

Under Layer Style(Layer > Layer Style) add an Inner Shadow, Gradient Overlay and Pattern Overlay blending options to the motion design layer.

Step eight:

Grab the original ‘Text Design’ layer set and place it on top of every layer on the layer window. Place it as shown below.
