Step one:
Start with a large black canvas. Make a new layer set titled Head Display.
Load your picture inside the canvas. We only need the face so erase the unesassary parts.
Step two:
In a new layer draw a black eclipse using the eclipse tool. (width: 83 x height: 112)
Add the following layer styles: Inner Glow, Gradient Overlay.
Set layer's blending mode to Lighten.
