Step one:
Start with a large black canvas. Make a new layer set titled Name tag.
Choose the rounded rectangle tool and set the radius setting to 40px and check anti-aliased.
In a new layer draw a black round rectangle. (width: 234 x height: 20)
Add the following layer styles: Inner Shadow, Outer Glow, Gradient Overlay, Stroke.
Step two:
In a new layer draw a #709F1C rectangle. (width: 22 x height: 20)
Add the following layer styles: Inner Shadow, Outer Glow, Gradient Overlay, Stroke.
