Step one:
Start with a large black canvas. Make a new layer set titled Red Glass Plate Header.
Set your foreground to #BB0000 and background to #8E0000.
In a new layer make a wide marquee selection. (width: 600 x height: 180)
Inside the selection, add a linear gradient effect using the gradient tool.
Step two:
Select the rounded rectangle tool and set the the radius setting to 20px with anti-aliased.
In a new layer draw a #BC0000 round rectangle. (width: 214 x height: 83)
In a new layer draw a #000000 circle. (width: 66 x height: 66)
Add the following layer styles: Inner Glow, Stroke.
